Cardiac anesthesia is a subspecialty of anesthesia, which is technologically expanding and attaining new heights. Worldwide there are few textbooks devoted to the subject. 'Clinical Practice of Cardiac Anesthesia' by Dr Deepak Tempe was the first endeavor into this field from the nonwestern part of the world. This popular textbook of Cardiac Anesthesia is now in its Third Edition. The book covers the various aspects of cardiac anesthesia very comprehensively. The book is today the most widely used clinical reference in cardiac anesthesia in India.

Thoroughly updated, this edition provides specific clinical recommendations for anesthesia pertaining to cardiac surgery. The book contains sections on anesthetic management for specific cardiac disorders, cardiopulmonary bypass, circulatory support, postoperative management, and perioperative support. The book also features numerous figures and tables to highlight key points, and the writing style is simple yet effective.

A new chapter on Cardiac Physiology has been added in this edition, which explains the nuances of cardiac physiology in a practical and easy to understand way. The book has very lucidly described the basics of trans-esophageal echocardiography (TEE), hemodynamic monitoring, and neurological assessment after cardiac surgery. The book has illustrated images of various lesions in cardiac disorders as seen in TEE, making it a useful tool for those not so well versed at using TEE. The present status of Off-pump Coronary Artery Bypass surgery and the role of Thoracic Epidural Anesthesia in cardiac surgery have been highlighted in this edition.

The book incorporates clinically relevant basic science into a practical "what-to-do" approach and is written in an easy-to-read outline format. It is well organized with 18 chapters covering the basics such as physiology and pharmacology, to congenital heart disease, valvular heart disease, coronary artery disease, and cardiac intensive care. Anesthetic aspects of management of patients with cardiac disease for noncardiac surgery are very lucidly covered. There is a special chapter on the role of the anesthesiologist in the Cardiac Catheterization Laboratory. There is also a chapter on blood conservation in cardiac surgery, a field usually ignored.

The management of cardio-pulmonary bypass has been described well. Myocardial protection has been given its due importance and presented as a full chapter. The monitoring section is clear, compact, and describes the utilities and limitations of various monitoring modalities. The high resolution intraoperative TEE pictures, carefully chosen, enhance the utility of the book.

The chapter on postoperative care covers the broad aspect of intensive care of postoperative cardiac surgery patient, including a short review of various modalities of mechanical ventilation. Finally the chapter on protocols gives a precise and concise guideline for the management of anesthesia for adult cardiac surgery. This chapter is a must and should be carried by relatively inexperienced cardiac anesthesiologists at all times .

In summary, this is an excellent introductory text book for a trainee in cardiac anesthesia and for a MD or DNB student of Anesthesiology, interested in pursuing sub-specialty anesthesia. The book is also a must read for cardiac surgeons, to improve their understanding of issues during the perioperative period.
